Enrollment Information and Forms

Congratulations on your decision to register your child at Fort Mill Preparatory Preschool! We understand how important this decision is, and know you have put a great deal of thought and effort into ensuring it is the right decision not only for you, but your child as well.

For your convenience, we are providing an Enrollment Application Form that can be completed  and then either mailed or faxed to our center. While some classrooms may have immediate space availability, others may be fully enrolled. Please call our center to check availability. If the classroom you desire is currently at capacity, this form ensures that your child will be placed on our waiting list and that we will contact you when space becomes available.

Our enrollment process is extremely easy! We ask you to do the following:

• Visit the center, meet the staff, tour the facility and ask questions!
• Visit your child's classroom, meet the teachers and ask questions!
• Sit down with a member of our Administrative Team and discuss your expectations of childcare, and find out what our goals are for you and your child!
• With a member of Admin., select a date for your child to begin.
• Complete a full Student/Parent Information Form name, address, emergency contact numbers, etc.
• Complete a State Licensing Health Statement Form for your child.
• For children under the age of 2: Complete an Infant Information Sheet.
• For all children: Complete a Transportation Agreement Form (used for children under 4 for emergency situations only).
• Complete an Emergency Medical Authorization Form.
• Sign a FMPS Discipline Policy (FMPS utilizes prevention, redirection, etc; corporal punishment is never allowed).
• Bring a copy of your child's Immunization Record from your Pediatrician (note this is extremely important - the State Health Department will not allow children to attend public care without on site records of their immunizations, or a note excusing your child from immunizations due to religious preference from the health department).
